Fit for Frailty best practice guidance

This Fit for Frailty best practice guidance has been produced by The Royal College of General Practitioners and recommends a move away from traditional ‘activity’ based contracts to a model that sees providers paid for helping people who use their services to achieve agreed goals.

The guidance says that this would ensure that outcomes relevant to older people with frailty, including quality of life, loneliness and pain are properly considered in the development of services.  The guidance also states that older people with frailty have the most to gain from health and social care integration and that joining up services and personal budgets is essential.
