Launch of new report on person-centred coordinated care
Think Local Act Personal (TLAP) and National Voices have published a new narrative in which children and young people with complex lives speak out on what they expect from good person-centred care that is coordinated around their needs.
My life, my support, my choice is endorsed by the national Children’s Health and Wellbeing Partnership and supports the implementation of the policy initiatives and legislation that are currently shaping services for children and young people.
For children with complex needs who may need to access multiple services, having care that is well coordinated -integrated- whilst positioning them at the core with a person-centred approach is fundamental. The guide offers a clear set of outcomes that commissioners and service providers should be working to, along with a powerful reminder of what children and young people want from their lives, not just from the immediate function of the services they receive.
